Passport Acceptance

Following King County recommendations to slow the spread of COVID-19, we are not accepting passport appointments until further notice.

 

October 12th, 2020 Update: At this time, some acceptance facilities are open for passport processing with a return time of 10-12 weeks. Expedited services have resumed and the return time is 4-6 weeks for an additional fee. It is encouraged to apply by mail when possible. The Sammamish City Hall remains closed, and is not accepting passports until further notice.

The Seattle Passport Agency in downtown Seattle is open for emergency situations only, with required proof of travel within 72 hours. Appointments can be made by calling the National Passport Phone Line at 1 (877) 487-2778.

do I need to Apply in Person?

You must apply in person using Form DS-11 if at least one of the following is true:

  • You are applying for your first U.S. passport
  • You are under the age of 16
  • Your previous U.S. passport was issued when you were under the age of 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

 

If none of the above statements apply, you must Renew by Mail using Form DS-82. The City of Sammamish is not able to accept the DS-82 form.

The City accepts passports by appointment only on Tuesdays & Fridays from 9 am - 1 pm.

 

If you are eligible to apply in person, please complete the checklist below to schedule an appointment. Please arrive at least 10 minutes prior to your appointment time as late applicants will not be accepted.

Passport Fee Information:

Passport fee is payable to: U.S. DEPARTMENT OF STATE (check or money order only).

Processing fee is payable to: City of Sammamish (cash, check, Visa and Mastercard).

Note: All applicants must appear in person. Notarized or unofficial photocopies of official documents (e.g. birth certificate, marriage certificate, adoption decree) are not acceptable. The documents submitted must be the original or certified copies from the issuing agency.

In addition, children under 16:

  • Must present evidence of parental relationship (birth certificate, adoption decree) in addition to citizenship evidence (passport, birth certificate).
  • Are affected by the Two-Parent Consent Law.
