March 1st, 2021 Update: The City of Sammamish City Hall is still closed to the public and we are not processing passport applications. There is no estimated date for the resumption of services at this time. Please visit https://iafdb.travel.state.gov to find an open facility near you. Please note, many acceptance facilities are dealing with a high volume of applicants and appointments may be limited.
October 12th, 2020 Update: At this time, some acceptance facilities are open for passport processing with a return time of 10-12 weeks. Expedited services have resumed and the return time is 4-6 weeks for an additional fee. It is encouraged to apply by mail when possible. The Sammamish City Hall remains closed and is not accepting passports until further notice.
The Seattle Passport Agency in downtown Seattle is open for emergency situations only, with required proof of travel within 72 hours. Appointments can be made by calling the National Passport Phone Line at 1 (877) 487-2778.

You must apply in person using Form DS-11 if at least one of the following is true:
If none of the above statements apply, you must Renew by Mail using Form DS-82. The City of Sammamish is not able to accept the DS-82 form.
The City accepts passports by appointment only on Tuesdays & Fridays from 9 am - 1 pm.
If you are eligible to apply in person, please complete the checklist below to schedule an appointment. Please arrive at least 10 minutes prior to your appointment time as late applicants will not be accepted.

Passport Fee Information:
Passport fee is payable to: U.S. DEPARTMENT OF STATE (check or money order only).
Processing fee is payable to: City of Sammamish (cash, check, Visa and Mastercard). Note: All applicants must appear in person. Notarized or unofficial photocopies of official documents (e.g. birth certificate, marriage certificate, adoption decree) are not acceptable. The documents submitted must be the original or certified copies from the issuing agency.
